Everything You Need to Know About Breast Augmentation

What Decision Do Dr. Hobar and I Need to Make?
- Type of implant: Dr. Hobar uses both saline and silicone implants, both with excellent results. Since Dr. Hobar prefers placing the implants underneath the muscle, both implants will look and feel the same on the upper half of the breast where the muscle is present. This means you will not be able to tell a difference in a bathing suit or a low cut dress. The benefit of saline implants is that, in the chance that there is a rupture, it is immediately noticeable, the body reabsorbs the saline, and the repair is easy. Saline is also a good option for women with asymmetrical breasts as different volumes can be added to each side. The benefit of silicone implants is that they can be more natural feeling in the lower half of the breast where the muscle does not cover the implant. This can be advantageous for thin women with little subcutaneous tissue.
- Incision Site: Dr. Hobar does use all three types of incision locations, inframammary fold (below the breast), transaxillary (through the armpit-see picture), or periareolar (through the nipple). Dr. Hobar prefers to use the transaxillary incision approach, as it leaves no unnecessary scar on the breast. He uses the inframammary fold if there is a previous scar there, or if a breast lift is needed as well.
- Implant Size: During the consultation, Dr. Hobar and you will discuss breast size. Dr. Hobar will listen to your wants as well as assess your body’s natural shape and available tissue support. Ultimately, he will recommend what size he thinks is best for your body, both in appearance and in future health. It is less about the exact cc volume of the implant, and more about how it will fit within your natural breasts and enhance your body shape. What to Expect From Start To Finish:
The first step is scheduling a consult with Dr. Hobar. In select situations, Dr. Hobar can provide an online consultation, in which you securely submit your information and photos and Dr. Hobar will give you an initial opinion. Together you and him will assess your desires and body type to determine if you are a good candidate for breast augmentation. Dr. Hobar will discuss the options you have and allow you to feel the different implant types. After the consult, Dawna our practice manager, will discuss cost, how to prepare for surgery, and surgery scheduling. Pre-operative photos will also be taken in the clinic for use of Dr. Hobar on the day of surgery. He performs surgery at Baylor Surgicare North Dallas under LMA anesthesia (your asleep but don’t have a breathing tube through your vocal cords), and typically blocks out around an hour and a half for the procedure. The procedure usually takes about 40 minutes, but Dr. Hobar always blocks out extra time. He has been working with the same surgical team for more than 15 years. After surgery, you will be discharged that day and can typically resume light activity the same day. The implant is typically placed behind the pectoral muscle, and while the muscle stretches to accommodate the implant there is some discomfort. Placing the implant under the muscle, in Dr. Hobar’s opinion, makes the implant look and feel more natural, puts less stress on the skin over time, and preserves the reliability of a mammogram. Muscle relaxants are prescribed to help with the accommodation process, as well as pain relievers to make you as comfortable as possible. Dr. Hobar recommends the patient resumes light exercise after about a week in order to speed up the recovery process. You will be seen a week following the surgery to discuss massaging and exercises to ensure the proper position of the implant.
