Dr. Hobar has championed the technique of cheek (midface) lift through a hidden incision in the mouth and the scalp. Dr. Hobar's technique and experience was presented in a video at the annual meeting of the American Society of Plastic and Reconstructive Surgery in October of 1998 and now will be archived in their video library. Dr. Hobar explains: "The middle portion of the face is a very key part of the face in the aging process and is often one of the first areas to show signs of aging. There is a triangular pad of fat that overlies the muscles in this region. When it is up high, as in youth, it adds to a tight look of the skin, helps enhance the cheekbones, and supports a youthful, fresh look of the lower eye region. As a person ages, this fat pad tends to descend with gravity, dragging the skin with it. This can result in prominence of the wrinkles around the corner of the mouth, a hollowness to the eyes, and a more sunken appearance of the cheekbones. The subperiosteal approach that I use is a very safe, effective way to restore a more youthful appearance to this region. I frequently perform this technique in combination with a facelift, but some patients are excellent candidates for this procedure alone. The technique is performed through hidden incisions inside the mouth and in the scalp. The elevation of the cheek pade produces a more youthful look around the mouth, in the cheek and around the eyes. It also frequently restores prominence of the cheekbone area that was present in youth.
